Plan before removing

One of the commonest requests is for a smaller, less bulbous nasal tip. Again I think one of the most important things about addressing the bulbous tip is to plan in advance why the tip is bulbous. There is absolutely not one size that fits all. Seeing a lot of patients for revision surgery, I see when tip surgery is performed badly and it’s because there’s an appreciation and thought, that one size fits all, that does not happen, you don’t want notched nostrils, you don’t want a pinched tip. You want a natural non-surgical slimmer nose.

Reshaping the nose

The reason that surgery is difficult and I think bulbous nasal tip surgery is difficult, is because you have to reshape the tip cartilages you can’t just cut them out. Disaster lies in cutting out the tip cartilages; I see it a lot. You need to reshape the nasal tip, you need to have at your disposal, an understanding of all the suture techniques all the grafting techniques. I have used them all and I think there are some things that work better than others, but again it is a matter of explaining to the patient what the deformity is, and how you are going to change it. That is the first step in getting it right and then executing the surgery well. I have done a lot of tip surgery, it is not straightforward, but it’s the tailored approach that matters more than anything else.
